This form is used for Course Approval. Forward this form by mail or email to Cathyk@okvetboard.ok.gov along with:
1. A published notice of the meeting.
2. A planned program as evidenced by a published agenda.
3. A formal presentation on printed material (ie: papers, brochures, videos w/printed material describing the video contents, etc)
After the meeting, submit a copy of the sign-in sheet which contains the attendees printed name and signature to the Board office.
This form is used to notify DEA that your drug registration will not be used in the future.
