Application packets and renewal forms for licenses isses by the OSBVME 

Renewal forms will be mailed in March each year and due by June 30th each year (July 15th for Faculty) - ONLINE SYSTEM OPENS APRIL 17, 2024

There are two options to renew; sending in a paper renewal or the online renewal process.  

 Again required this year - one hour of Opioid CE required - can be for pain management in opioid use or addiction -please call if you  have questions

 If you have an OBNDD registration and the number is more than five digits - delete the 100 and add last five digits

As of 2022 NO Online Certificates are required - please keep all certificates in case of  an audit

Online Renewal  closes  from September to Mid April every year - please do not enter information into system while Online Renewal is closed
